The medical home team and the PCBH Provider will work together to establish a comprehensive support and service system within the patient’s medical home.   • To develop patient-centered behavior change plans, the PCBH Provider will implement concise evidence-based treatment interventions and best practices, including motivational interviewing, cognitive behavioral strategies, and motivational education.
  • This role involves providing care to patients of all ages, stages, and capacities, as well as families and groups.
  • The role necessitates consultation, education, and collaboration with internal and external healthcare teams, as well as other community and referral organizations.
  • The patient’s identified treatment objectives may necessitate additional care requirements, which are within the scope of PCBH and the professional responsibilities of the registered nurse or social worker.
  • You will be expected to effectively integrate into the culture and tempo of Medical Home co-location, with the support of colleagues from both within and outside the organization.

Who Should Apply for Primary Care Registered Nurse Jobs in Calgary?

  • Bachelor of Science in Nursing, Registered Psychiatric Nurse, or Social Work.
  • Enrollment in the appropriate institution, as authorized by the Health Professions Act.
