We are seeking a dedicated and compassionate Health Care Assistant (Band 2) to join the Discharge Lounge Team at St. Helier Hospital. This role is essential to ensuring that patients awaiting discharge receive high-quality care and support in a safe and well-managed environment. You will work closely with the multidisciplinary team to coordinate discharges, maintain accurate documentation, and support both patients and families during transitions.

Duties and Responsibilities:

  • Facilitate the safe and timely transfer of patients from wards to the Discharge Lounge.
  • Accept clinical handovers from ward staff and advocate for patient movement to ensure efficient bed flow.
  • Provide personal care and support to patients within your level of competence and training.
  • Ensure a clean, safe, and welcoming environment in the discharge area.
  • Maintain accurate and up-to-date patient records related to discharge.
  • Coordinate transport and escalate delays to relevant teams promptly.
  • Communicate with Clinical Site Managers, ward staff, and allied health professionals to support effective discharge planning.
  • Assist patients and their families by offering relevant information and responding to enquiries in a courteous and respectful manner.
  • Support clerical and general reception tasks as required.
  • Uphold the dignity, privacy, and confidentiality of patients at all times.
  • Participate in cleaning duties and use Trust IT systems when required.
  • Undertake patient observations following appropriate training and competency assessment.
