We are seeking a dedicated and compassionate Health Care Assistant (Band 2) to join the Discharge Lounge Team at St. Helier Hospital. This role is essential to ensuring that patients awaiting discharge receive high-quality care and support in a safe and well-managed environment. You will work closely with the multidisciplinary team to coordinate discharges, maintain accurate documentation, and support both patients and families during transitions.
Duties and Responsibilities:
- Facilitate the safe and timely transfer of patients from wards to the Discharge Lounge.
- Accept clinical handovers from ward staff and advocate for patient movement to ensure efficient bed flow.
- Provide personal care and support to patients within your level of competence and training.
- Ensure a clean, safe, and welcoming environment in the discharge area.
- Maintain accurate and up-to-date patient records related to discharge.
- Coordinate transport and escalate delays to relevant teams promptly.
- Communicate with Clinical Site Managers, ward staff, and allied health professionals to support effective discharge planning.
- Assist patients and their families by offering relevant information and responding to enquiries in a courteous and respectful manner.
- Support clerical and general reception tasks as required.
- Uphold the dignity, privacy, and confidentiality of patients at all times.
- Participate in cleaning duties and use Trust IT systems when required.
- Undertake patient observations following appropriate training and competency assessment.
