We are in search of a registered nurse to join our Home Living team and offer professional support services to clients of all ages and phases in their residences. The registered nurse is a self-directed team member responsible for ongoing care. In this role, they are responsible for determining the client’s needs, providing therapeutic nursing services in accordance with professional practice standards, coordinating resources, establishing objectives for the delivery of services, and assessing the results.

Furthermore, this role requires the direct provision of client care that is client-centered, evidence-based, and compliant with acknowledged clinical standards, as well as the case management of clients. The registered nurse functions as a consultant on the team, offering clinical guidance to other team members relevant to their respective cases. The direct RN plans care in collaboration with the client and case manager to address the client’s unmet nursing requirements. This is achieved by utilizing the appropriate internal and external resources and conducting nursing assessments and interventions.

This is a designated auxiliary position that provides support to Southeast Home Care. The successful candidate may be scheduled to work a variety of shifts and will be assigned to specific geographical teams to provide primary coverage for open shifts, based on the need.
